Referral Process

Step 1: Submit Referral

Choose the option that works best for you:

What We Need:

  • Client name, date of birth, contact information
  • Brief clinical summary (diagnosis, substances used, treatment history)
  • Current medications (especially MAT)
  • Insurance information
  • Level of care recommendation (PHP vs IOP)
  • Any safety concerns or special needs


Step 2: Insurance Verification

We verify benefits within 24 hours and contact the referring provider with coverage details and out-of-pocket costs for the client.

Step 3: Schedule Intake

First appointment scheduled within 48 hours. Clinical assessment conducted, treatment plan developed, and client begins programming.

Step 4: Ongoing Communication

Progress updates sent to referring provider (with ROI). Discharge planning coordinated in advance. Warm handoff to next level of care or community supports.
